🌡️Insulin Storage & Beyond-Use Dating (BUD)
Improperly stored insulin loses potency — and you may not even know it until blood glucose is unexpectedly high. This guide covers every rule for storing vials and pens correctly, from pharmacy to travel.
Why Insulin Storage Matters
Insulin is a delicate protein hormone. Exposure to extreme temperatures, direct sunlight, or prolonged room-temperature storage can cause insulin to degrade — breaking down the protein structure so it loses some or all of its glucose-lowering potency.
Degraded insulin may look unchanged (clear for analogs, or properly mixed for NPH) — there is often no visual sign that it has been damaged. The result is unexpected hyperglycemia, DKA risk, and wasted medication.
Unopened Insulin: Refrigerator Storage
All unopened insulin — vials, pens, and cartridges — should be stored in the refrigerator at:
- Store away from the freezer compartment and any vents that produce direct cold air
- Never freeze insulin — freezing irreversibly damages the protein crystals
- Unopened, refrigerated insulin can be used until the manufacturer's expiration date printed on the label
Opened Insulin: Room-Temperature Storage (In-Use)
Once a vial or pen is opened (first punctured or the cap removed), it is designated "in-use" and subject to Beyond-Use Dating (BUD) rules — a fixed number of days after which the insulin must be discarded, regardless of how much is left.
In-use insulin can be stored at room temperature (below 77°F / 25°C). Injecting cold insulin directly from the refrigerator can be uncomfortable and may alter absorption rates. Many patients keep their in-use vial at room temperature for comfort.

Note: BUD values are manufacturer-specified and FDA-reviewed. Signs of Damaged Insulin
- Clear insulin (analogs, regular) that appears cloudy or has particles — discard immediately
- NPH/premixed insulin that doesn't re-suspend after gentle rolling — clumping or particles that don't dissolve → discard
- Any insulin that has been frozen — discard, even if it looks normal after thawing
- Any insulin stored above 77°F (25°C) for extended periods — potency may be reduced even without visible changes
- Unexpected high blood glucose with no other explanation — always consider insulin degradation as a cause
Traveling with Insulin
Air travel:
- Always carry insulin in your carry-on luggage, never in checked bags — cargo holds can freeze at altitude
- TSA allows insulin, syringes, lancets, and meters through security — no special container required, but a clear bag helps
- Bring a signed letter from your prescriber for international travel
- Crossing time zones: maintain the same number of hours between basal doses rather than switching suddenly — discuss with your care team
Hot climates:
- Use an insulated insulin travel case or Frio evaporative cooling wallet — keeps insulin at safe temperature for 45+ hours without refrigeration
- Frio wallets work by evaporation — wet the wallet and the cooling crystals inside keep the pouch cool as water evaporates
- In extreme heat (>95°F / 35°C), refrigerate at the hotel when possible and use the cooling wallet only when on the go
Insulin Pump Reservoir & Infusion Set BUD
For insulin pump users, the insulin in the reservoir and the infusion set (tubing + cannula) also have BUD limits:
- Reservoir insulin: Most manufacturers recommend changing every 2–3 days with the infusion set change. Rapid-acting analogs in a pump (body temperature, movement) degrade faster than in a vial.
- Infusion set/site: Change every 2–3 days to prevent lipohypertrophy, infection, and occlusion.
- Many pump users notice higher blood glucose on day 3 of a site — this is often a sign the site should be changed sooner.
